Trans*-positive GP near South Kensington?

OKAY SO. I hope & expect to be moving to London in the autumn. I have got an endometriosis specialist sorted (or will do shortly, at any rate); getting transferred to a London pain clinic should also be pretty easy.
However! That still leaves me in need of a GP.
I need a GP who:
I would really, really love recs, so I don't have to sort this all out by trial and tedious error. Please do feel free to signal-boost also, & in any case have my eternal gratitude!

I need a GP who:
- is trans*-positive, and preferably known good at handling genderqueer people
- is not phased by patients with multiple interacting complex conditions
- is willing to work with patients who are proactive about managing their health (i.e. isn't going to refuse to treat me because I self-medicate carefully & with sound logic)
- is, ideally, in the South Kensington area (because public transport barriers to seeing GPs means I'm much less likely to... see GPs when I need to, and that's a pity)
